As a state-authorized Aging Services Access Point (ASAP), Ethos coordinates all the non-institutional home and community-based services for elderly and disabled residents of the southwest Boston neighborhoods of West Roxbury, Hyde Park, Roslindale, Jamaica Plain, and Mattapan and well as, provides city-wide nutrition services in Boston, which serve approximately 2.1 million meal for older adults, delivered to the home (Meals on Wheels) or served in neighborhood congregate settings (Community Cafés). Ethos is looking for a full-time Registered Dietitian to join the Nutrition Department. The dietitian is primarily responsible for performing nutrition assessments and education, training staff as it relates to nutrition, conduct kitchen inspections of meal vendors, and reviewing NSI forms for home bound elders. The dietitian will implement nutrition-based interventions and follow up with elders to achieve documented outcomes through home visits. Additionally, the dietitian will develop, present and evaluate evidence based nutrition education for congregate meal site participants. Calculate nutrient composition of menus and review as needed with Nutrition Program Director.
DUTIES:
1. Review client SIMS record and/or nutrition screening referrals.
2. Complete nutritional assessments of consumers using proper documentation forms.
3. Develop, implement and monitor nutrition interventions and outcomes for consumers. This includes contacting family, case manager, health practitioners (nursing, primary care physicians, dialysis centers, etc.), Protective Services, etc.
4. Provide nutrition counseling to appropriate nutritionally at-risk clients and work with clients to implement nutrition plans.
5. Perform follow-up assessments necessary to monitor effectiveness of nutrition plan.
6.Document assessment, care plan and follow-up in client record.
7. Implement and record outcome documentation to measure the effectiveness of the nutrition intervention.
8. Review caterer menu, calculate nutrients using Computrition. Complete menu from AGE.
9. Attend nutrition meetings as scheduled. Attend multi-team conferences and present case studies as necessary.
10. Assess needs, develop nutrition education programs, present and evaluate same at congregate nutrition sites.
11. Complete monthly statistics as requested.
12. Complete annual quality survey of clients who have received nutrition intervention.
13. Perform staff sanitation trainings as needed.
14.Perform other related duties as assigned by supervisor.
QUALIFICATIONS:
1. Registered Dietitian.
2. 3 to 5 years of experience as Nutritionist; 1 to 2 years of community nutrition experience. Experience with elder population preferred.
3. Active member of the Academy of Nutrition and Dietetics preferred.
4. Must be able to drive or travel in an efficient manner.
5. Must be able to traverse at least two flights of stairs.
6. Intermediate computer skills, spreadsheets and word processing
7. Acceptable CORI report at hire and ongoing.
