Summary: Malnutrition is prevalent among hospitalized patients and those living at home, and often goes unrecognized across health care settings. Malnutrition results from an imbalance of nutrient intake, influenced by acute and chronic illnesses or social environmental factors. Malnutrition can impact patient’s health outcomes, hospital length of stay, readmission rates, and disease recovery. These are crucial reasons for RDs to be involved in patient care plans not only during hospitalization but thereafter. This presentation is intended to discuss some of the barriers to including malnutrition status in transitional care planning and the role RD’s can play to help bridge this gap.
Cost: $20 for MAND members; $25 for NON-MAND members $10 for students.
